Hospice Care Settings
AseraCare Hospice offers hospice services in a variety of locations – patients’ homes, hospitals, nursing homes and assisted living facilities. We believe it is important for hospice patients and their families to be in comfortable and familiar settings, wherever they choose to receive their care.

Home

Many patients prefer to receive hospice care at home. This
allows them to remain in familiar settings, surrounded by friends
and family, while receiving the care they need. Based on an initial
assessment by the AseraCare Hospice team, we will design a care
plan that can include either regular, intermittent care, or
continuous care 24 hours a day, all in the comfort of the patient’s
own home.
Nursing homes

AseraCare Hospice provides hospice care to residents in skilled nursing facilities (SNF). Our Hospice-SNF Coordination of Care program creates partnerships between our hospice teams and nursing home staff to provide seamless coordination of care. Nursing home residents who choose hospice care benefit by staying in a familiar setting, with caregivers they already know, plus access to hospice professionals who can recognize how individuals express pain and can suggest a number of ways to help control it.
Assisted living

AseraCare Hospice provides hospice care to people who need help with activities of daily living, but have chosen to live as independently as possible, for as long as possible. When a resident of an assisted living facility chooses hospice care, our interdisciplinary hospice team supplements the care provided by the assisted living staff. AseraCare Hospice staff is available to answer questions and handle emergencies, 24 hours a day, seven days a week.
